The Social Prescribing Service is available to everyone over the age of 18 in North Tyneside.
You can be referred into The Social Prescribing Service by your GP or other NHS health professional, or you can refer yourself by filling in a referral form and sending it to us. We will then send it to your GP to be stamped.

Once we receive your stamped referral form, we will arrange a face to face assessment which will make sure that we can meet your needs, and give you the chance to talk about what kind of things you are interested in, or need help with.
We will arrange for 10 funded sessions of an activity for you, and come to the first session with you if you need us to. Your coordinator will be there at the end of a phone throughout your time with us, to ensure that you feel supported. At your last activity session, your coordinator will meet you to get some feedback about how you have found your time with the Social Prescribing Service – we aim to improve your sense of wellbeing by supporting you to be involved with the activity of your choice, and we hope that this positivity carries over into other areas of your life.
We are not able to pay anything towards any travel costs.
