The Social Prescribing Service is a partnership between Age UK North Tyneside, the Percy Hedley Foundation and Tyneside MIND and is designed to support individuals by linking people up to social or physical activities in their community, to improve their overall health and wellbeing.
The service is available for anyone over the age of 18 in North Tyneside with a wide variety of conditions, which can include stress, depression, anxiety and dementia, and is available to people with mental health problems, autism, learning disabilities, physical disabilities, older people, people in transition and people with long term conditions.
The service is personalised, flexible, adaptable, reliable and responsive to your needs.

If you would like to take part in Social Prescribing, ask your GP or NHS healthcare professional to complete a referral form, or complete one yourself and we will send it to your GP to be stamped.
